How Do I Get In?

The youth in the Transitional Living Program have to be 17 years old, be a current State Ward, or have been a State Ward on their 18th birthday, and have the motivation to follow the rules and work hard to be able to live on their own. Referrals often come from case managers, therapists, PALS workers, or even their own family members. A two-page referral form is used to begin the interview process and is available to be filled out by clicking the link below. For more information, contact the Director using the email link below.

Christian Heritage provides all of the furniture and supplies for the apartments, so donations of furniture, dishes, vacuums, and many other essentials are always needed. Again, community support makes this program possible by meeting the tangible needs of the youth in this program.
